ACCESS SERVICE

​YOUTH SUPPORT PROGRAM​​

It’s not always easy being young! - trying to balance family responsibilities, education, social life & relationships can be stressful.


Our youth support program assists young people aged 8 - 21 years in a holistic case-management framework, working closely with other agencies to deliver innovative and effective services to at-risk and disadvantaged young people.

Referrals to the program includes young people who are:

 

  •   At risk of disconnection from family/community or support networks

  •   At risk of disengagement from school, training and/or employment

  •   At risk of harm, including self-harm

  •   Homeless or at risk of homelessness


Our youth workers aim to support young people by encouraging positive connections and communications with family, friends and community.

This support is client focused and client centred where young people actively participate in identifying their support needs including individual support, tailored activities, lifestyle education, community participation and advocacy.


The young person can contact Queensland Youth and Families Support Services (QYFSS) themselves, or be referred by service providers, family members, schools, friends, police or government agencies by phoning (07) 3818 1050 or completing the Online Referral Form.

The youth support service is funded through the Department of Child Safety, Youth and Women.

Access Services assist individuals and families to connect with available supports, and is delivered through information, advice and referral.  

 

Access services can also link young people to community-based activities and events.

SUPPORT SERVICE

Support service is delivered through advice, information, individualised case management or case coordination, and referrals to other local services such as Intensive Family Support (IFS) and Family Wellbeing (FWB) services.

 

Queensland Youth and Families Support Services have formed collaborative partnerships with specialised service providers within the local region to ensure the best outcome for the young person and their families.
